Our Youth Case Management service offers practical, compassionate support designed to help you navigate life’s challenges, find your voice, and take positive steps forward. We take the time to listen, understand what matters to you, and work with you to set goals and achieve meaningful outcomes. Support is flexible, confidential, and tailored to your situation.

What you need to know
Case management is a structured, supportive process designed to help young people who are experiencing challenges or vulnerabilities. Our Youth Support Specialists work with you to understand your needs, develop a personalised plan, and ensure you have access to the right help at the right time.
Young people aged 12-24 living in the Fairfield, Campbelltown and Liverpool areas.
You can reach out directly, or be referred by a school, health worker, or other service provider. Either way, we’ll guide you through the next steps.
Support is flexible and based on your needs. This may include individual case management, mentoring, help accessing services, advocacy, or connecting you with programs and opportunities that can help.
Yes. Everything you share with us is treated with respect and kept confidential. We only share information with your permission or when required by law to keep you or someone else safe.
The service is funded by the Department of Communities and Justice Targeted Early Intervention Program.
